Quest Investimentos Ltda. neither trades nor distributes shares of investment funds or any other financial asset. QUEST´s fund management is carried out by Mellon Serviços Financeiros DTVM S/A. Quest´s funds are distributed by Mellon Serviços Financeiros DTVM S/A, Link S/A CCTVM and other firms.

The list of registration form documents required by the Manager is available below. Download the files as directed:

Individuals
Copy of ID card (RG), Individual Taxpayer ID No. (CPF) and proof of address (cell phone bills not accepted) of the title holder or joint-holder(s).

Fill out and sign:

>> Statement of compliance (portuguese)
>> Signature card with a notarized signature/bank guarantee (portuguese)


Corporations

After filling and signing the forms, reading carefully the Rules and Prospectus of the fund in which you want to invest, send all the required documentation to the fund´s distributor:
